How do I find out who is leaseholder (and address) for a flat?

Look on http://www.landregistry.gov.uk . There is a lot of useful data on land and property, much of it free. If the lease has altered since 2000, it is very likely available for download. It costs £4 for a copy of the title register and £4 for copy of the title plan.

Check the land registry, or just you could just knock on the door and ask :)
